Obtaining a copy of your MCH medical records is easy:

1. Download the Medical Records Request form by clicking here

2. Mail, fax or email the completed and signed form to:

Requests will be processed within 15 business days. We will mail your records to the address specified on the authorization form, or you may pick them up at our office.
