The Social Services Department at Mountains Community Hospital provides a full range of services for our patients, residents, community, and families.
Our staff is composed of highly professional, health care team individuals. All discussions and records are confidential within our health care team as we are committed to our patients’ right to confidentiality. Social Services will assist patients and families in discharge planning for continued care after hospitalization, to include home health care, nursing
home placement, rehabilitation placement, hospice, durable medical equipment, and transportation.
Additionally, Social Services can provide supportive counseling for loss and bereavement, coping with stress, depression, anxiety, and role changes.
Services Offered:
Car Seat Program:The Mountains Community Hospital Car Seat Program provides car seats for infants up to twenty (20) pounds;
based upon necessity.
Pastoral Care Program: Pastors provide visits to all units and are available for crisis and/or disasters.
Layette Project: Layettes are donated for new mothers in need. Layettes contain clothing, blankets, etc., for at least the first six weeks for a newborn.
Family Violence and Outreach Program (FAVO): This program provides crisis intervention, support services, education/prevention, emergency housing, transportation, food, court support, and referrals to medical, legal, and counseling services.
