Welcome
to Mountains Community Hospital Rehabilitation Services!
We are a full service, multi-specialty team featuring
physical, occupational and speech therapy services, provided
by licensed and highly skilled therapists. Our services
provide active, progressive, comprehensive, individualized
therapy treatment with a personal approach to meet each
patient’s individual goals and health needs.
Mountains Community Hospital Rehabilitation Services is licensed
by the Department of Health Services and accredited by the Joint
Commission Accreditation of Healthcare Organizations. We are
located on the first floor of the Lake Arrowhead Medical Office
Building adjacent to Mountains Community Hospital, Suite 106,
where services are provided to our outpatients. Additionally,
we provide services to inpatients on the MCH Medical-Surgical
Unit, residents in the MCH Extended Care and Recovery Center,
and contracted home healthcare agencies that provide home healthcare
services to our mountain residents.

Community Hospital Rehabilitation Services has comprehensive
rehabilitation care you need, right around the corner in the
beautiful mountain community of Lake Arrowhead -- Approximately
25 miles from Junction Hwy. 30 and Hwy. 18 (Waterman Avenue).
Our Staff is available to answer any questions you may have
regarding our services.

Hours: Weekdays, Monday through Friday from 7:30 a.m. to 6:00
p.m. by appointment.
