A “Celebration of Don’s Life” is being held on June 13, 2010 at the Mountains Community Hospital from 2:00 PM to 4:00 PM. Don was a local doctor with many patients and friends. All are invited to attend to remember his life, his many good works, and the memories we all have.
In tribute to Don’s caring for his patients and the benefits they derive from the Mountains Community Hospital, you are invited to make a contribution in Don’s name to the “Mountains Community Hospital Foundation”. You may make a contribution by check or by e-mail:

Make the check payable to “Mountains Community Hospital Foundation” and write “In Tribute to Doctor Donald D. Doty” in the memo area on the lower right of the check.
You may mail the check directly to the “Mountains Community Hospital” or bring it with you when you come to the “Celebration of Don’s Life” ceremony on June 13, 2010.

Click on the button “Contribute Online”, then the button “Credit Card/Paypal”, complete the required information including “Donations For” section. Use the “down arrow” to open the selections and click to select “In Memory of Doctor Doty”. When all information is complete and correct, click “Submit”.
